What do I need to do to become a patient of Independent Family Doctors?

Anyone may receive care at our clinic, simply by scheduling an appointment at (701) 234-9400. The most seamless way, however, is to have your medical records transferred to our clinic ahead of time. To do this, simply contact your previous provider and request a release form. Signing a release form will transfer your complete medical records to Independent Family Doctors. We also recommend that you contact your insurance company to notify them of the change.

Feel free to stop in, or call Independent Family Doctors at (701) 234-9400 with additional questions or concerns. We thank you for your continued support.
